Houston Community Health Improvement Plan (CHIP) 2018-2021 Priorities
The Houston Health Department Community Health Improvement Plan 2018-2021 is the product of collaboration between community partners, community members, and the City of Houston, to improve the health and well-being of Houstonians. The CHIP is a long-term, systematic, community-driven plan to address five priority health areas identified in the Community Health Assessments and HHD Strategic Plan. All facets of the community have a role in applying the plan, in improving health, and quality of life of Houston area residents.
