Houston CHIP Process
The Houston Community Health Improvement Plan (CHIP) process is a comprehensive community approach. The process entails using a modified Mobilizing for Action through Planning and Partnership (MAPP) as the framework to develop the Houston Community Health Improvement Plan (2018 – 2021). The process engaged the community and aims to address considerations of social determinants by engaging partners and community members, focusing on resources and information available to help improve Houston's health together.
